What this worksheet is
Most families have plenty of numbers and no baseline, because each one was taken a different way on a different day. This worksheet fixes that: a definitions card for the five numbers plus the movement tests from an assessment, a baseline page with five trials and an average, a retest log for the end of each 4-week block, and plain-language rules for what counts as real change.
What's inside
- The rule: same tool, same setup, same conditions, same warm-up
- The metrics, defined: five numbers plus the movement tests from an assessment
- Baseline page: five trials and an average for every metric
- Retest log: weeks 4, 8 and 12, each with a change column
- What counts as real change: plain rules, no invented threshold
How to use it
- 1Fill in the tool, setup and conditions before the first trial.
- 2Log five trials and the average for each metric; that is the baseline.
- 3Retest at the end of each 4-week block and write what changed in training.
- 4Bring the sheet to the assessment, or to your next re-test if you already train with M4P.
When you want the plan written for your athlete
This worksheet is the starting point. An assessment measures where your athlete is today and turns it into a written plan.
